McKinney-Vento Homeless & Unaccompanied Homeless Youth
The form below is used to refer students and families who may be experiencing homelessness. It allows the Homeless Liaison to determine eligibility under the McKinney-Vento Act.
- Include all school-aged children.
- Also include children ages 0–5 and youth ages 16–18 who are not currently enrolled in school and have not graduated.
- Indicate any services needed (e.g., school supplies, free meals, transportation).
Once completed, the form is submitted through the online platform and shared with the Homeless Liaison for review.
