Homeless Eligibility Questionnaire
The forms below are to be used when enrolling new students who are homeless or when a school becomes aware that a family has become homeless.
We must document any children in the family 0-18 even if not currently enrolled in school (less than school age or have dropped out).
We must also document any services we provide to the students (school supplies, free school meals, transportation).
A copy of each form is sent to Dr. Gregory Monroe, Student Services Director, for state documentation purposes.
